Summary
Hidradenitis suppurative (HS) is a severe acne like disease which can impair quality of life considerably. We report on two patients with focal HS (grad 1 according to Hurley) who received botulinum toxin A in the axilla (250 Dysport U per site). Under this treatment an up to 12 month lasting remission could be achieved. Our results are in accordance with previous reports on the beneficial effects of botulinum toxin A for HS. Botulinum toxin A might be an alternative treatment in localized HS. Further studies should be performed to obtain better evidence for this indication.
Zusammenfassung
Hidradenitis suppurativa (HS) ist eine schwere dem Formenkreis der Akne zugeordnete Erkrankung, die die Lebensqualität deutlich beeinträchtigen kann. Wir berichten hier über zwei Patienten mit lokalisierter HS (Grad 1 nach Hurley) deren Erkrankung unter der Therapie mit Botulinumtoxin A (250 Dysport E pro Axilla) bis zu einem Jahre stabilisiert werden konnte. Unsere Beobachtung ordnet sich in den Kanon der bisherigen Fallberichte ein. Die fokale Therapie mit Botulinumtoxin bestätigt sich als eine mögliche therapeutische Alternative der lokalisierten HS. Zu wünschen wären weiterführende klinische Studien um die Evidenz zu dieser Therapieoption der HS zu stärken.

Konklusion
Zusammenfassung Die fokale Therapie mit Botulinumtoxin A stellt eine Option für die lokalisierte Hidradenitis suppurativa dar. Bei Verwendung von Dosierungen, die sich an der Hyperhidrosebehandlung orientieren, ist eine mehrmonatige Wirkdauer möglich. Wie alle Therapien, die bisher nur in Einzelfällen beschrieben worden sind, wären hier auch klinisch kontrollierte Studien wünschenswert, um Wirksamkeit und Sicherheit dieser Therapieoption zu untersuchen. Hypothetisch bleibt die Rolle dieser Therapie als Prävention zur Verhinderung einer Chronifizierung und ggf. auch einer weiteren Progression der Erkrankung.
